Canoe Trails Consent & Permission for Treatment

This form is to give consent on behalf of the participant
 for the legal authority to seek necessary medical treatment for any
type of injuries incurred during any event held by Canoe Trails. 
This document also releases all members of Canoe Trails, 
Canoe Trails, the Boy Scouts of America, and all other participating
 organizations from damages of any type. This document must be
  turned in to a CT Committee member by the designated due date. 
The Consent & Permission for Treatment document will not be
accepted by means of the Canoe Trails Forum or by email. Filling
out this documentation and turning it in does not guarantee a spot in
any activity or group going to Canada due to each individual
being reviewed and approved by the Canoe Trails Committee.
