Canoe Trails Health & Medical Record

This form is to document the current state of health
in which the potential participant is in. This document 
must be turned in to a CT Committee member by the designated
 due date. The Health and Medical Record document will not
 be accepted by means of the Canoe Trails Forum or by email. 
Filling out this documentation and turning it in does not 
guarantee a spot in any activity or group going to Canada 
due to each individual being reviewed and approved by
 the Canoe Trails Committee.
